Terms of Service
SonoMe Diagnostic Medical Inc., “we”, “our”, or “us” refers to SonoMe Diagnostic Medical Inc., a California ultrasound imaging services provider, and “you”, “your”, or “patient” refers to the person receiving services. By scheduling, attending, or engaging our services you agree to the following Terms of Service.
1. Services
We provide diagnostic ultrasound imaging and related services (the “Services”). Our scope includes ultrasound scans for obstetric, pelvic, abdominal, vascular, breast, renal, bladder, small parts, etc, interpretation and reporting by qualified personnel, and distribution of results to you and/or your authorized referring provider.
2. Appointment, Cancellation and Fees
You agree to provide accurate and complete information for scheduling.
If you cancel or reschedule outside our required notice period (48 hours prior to appointment time) you may be subject to a cancellation fee of $50.
Payment: you are responsible for payment of any fees not covered by your insurance or referral arrangement. We reserve the right to collect payment at the time of service if required.
3. Referrals and Insurance
If you were referred by a healthcare provider, we will coordinate with that provider as needed.
If you present to us without a referral (where required), you may be responsible for costs.
We will bill insurance as appropriate; you must provide correct insurance information.
4. Patient Responsibilities
Provide truthful, accurate, and complete medical history and information relevant to the imaging.
Follow instructions (e.g., fasting, preparation) for the imaging procedure.
Inform us of any changes to your health status, medications, pregnancy status, or implants/medical devices.
5. Imaging Results & Interpretation
The results of the ultrasound will be interpreted by a qualified professional and a report will be generated.
The imaging study is intended to assist in diagnosis or monitoring; it does not substitute for a full medical evaluation by your referring provider.
We will provide the report to your referring provider and/or you (as per your authorization).
In the event of incidental findings or urgent abnormalities, we will notify the referring provider promptly; you may also be notified as appropriate.
6. No Guarantee of Outcome
We perform the Services to professional standards, but we do not guarantee a particular result or outcome (for example, detection of every abnormality). Imaging limitations, patient anatomy, technical factors, and other variables may affect quality and findings.
7. HIPAA, Confidentiality and Privacy
We maintain administrative, physical, and technical safeguards to protect Protected Health Information (“PHI”) in accordance with HIPAA, CMIA and other applicable laws. For more detail please see our Privacy Policy.
8. Termination of Service
We reserve the right to decline or terminate services if you fail to comply with policies, provide inaccurate information, or if continuing the service would pose safety risks or violate law.
9. Limitation of Liability
To the maximum extent permitted by law, SonoMe Diagnostic Medical Inc., its officers, employees, contractors, or agents shall not be liable for indirect, special, incidental or consequential damages (including lost profits or business interruption) arising out of or related to the Services. Our total liability for any claim related to the Services shall be limited to the amount you paid for the Services.
10. Governing Law and Venue
These Terms of Service are governed by the laws of the State of California. Any legal actions shall be brought in the state or federal courts located in the County of Riverside, California.
11. Amendment
We reserve the right to amend these Terms from time to time. Notice of changes will be provided on our website and/or in-office. Continued use of our Services after notice constitutes acceptance of the amended Terms.
12. Severability
If any provision of these Terms is held invalid or unenforceable, the remaining provisions shall continue in full force and effect.
